I think that Ancient Cosmology graphic is incorrect. There are only three heavens. The air, the firmament (where the stars, wandering stars, sun and moon are) and Gods dwelling.
When Paul was caught up to heaven as described in 2 Corinthians 12:2, he was caught up to the third heaven.
